(Bottom of Page 4 on RFP) Proposer’s Certification Sheet Attachment 2, Proposer’s Certification Sheet, which shall be a part of the proposal package, must be signed by an individual(s) who is authorized to bind the Proposer contractually. An unsigned or incomplete Attachment 2 may cause the proposal to be rejected. For your Proposal to be responsive, all required attachments/exhibits must be returned with the Proposal; Proposals submitted that do not include the proper “required attachments” may be immediately rejected. b. Order of Required Attachments The required attachments should be in the same order as listed on Attachment 1, Required Attachments Checklist. All attachments which require signature must be signed by an individual(s) who is authorized to bind the Proposer contractually. Proposal Questionnaire Attachment 4, Proposal Questionnaire, is intended to provide CAAM with specific information concerning the firm’s capability to provide services as described in this RFP.
